We are looking for an individual who is passionate about hospice care and committed to providing exceptional support to patients and their families. As a Medical Social Worker, you will have the opportunity to make a meaningful impact by helping patients and caregivers navigate emotional, social, and practical challenges of end-of-life care. This is a PRN position with no guaranteed schedule or hours.

POSITION SUMMARY

The Medical Social Worker is a Master's or Bachelor's degreed professional who provides direct and indirect care services to patients in their home environment or alternative settings, according to the organization's policies and procedures. The MSW works to improve or maintain each patient's social, emotional, functional, and physical well-being while supporting caregivers and family members. This role collaborate with the interdisciplinary team and utilizes available community and agency resources to help patients achieve their plan of care goals.

QUALIFICATIONS

Have a Master's or Bachelor's degree in social work from an accredited school by the Council on Social Work Education, required. Have a current, valid Kansas State Social Worker license. Have hospice experience, within the past 3 years preferred. Have a minimum of 2 years of experience as a social worker, preferred. Possess a valid driver's license, reliable transportation, and auto insurance. Have or obtain a current CPR certification.
