• Regular, Full-Time | Monday-Friday, 8:00 AM-4:30 PM Pay Grade 12 (Salaried) | Compensation based on LCSW experience Weekly Earned Wage Access Available The Choctaw Nation of Oklahoma is seeking a Licensed Clinical Social Worker (LCSW) who is passionate about improving patient outcomes by removing barriers to care, coordinating successful transitions across the continuum of care, and advocating for patients and families during critical healthcare decisions. In this role, success is defined by your ability to assess complex psychosocial needs, connect patients with appropriate resources, facilitate safe and timely discharges, and collaborate with interdisciplinary teams to achieve positive clinical and quality outcomes. The ideal candidate combines strong clinical judgment, compassionate advocacy, and exceptional care coordination skills to support patients throughout their healthcare journey. What You'll Accomplish Within your first year, you will: Conduct comprehensive psychosocial assessments that identify social, emotional, behavioral, environmental, and financial factors impacting patient care and recovery. Develop and implement patient-centered care plans that improve access to services and support successful treatment outcomes. Coordinate safe, efficient, and timely discharges while reducing barriers that may delay transitions to home, rehabilitation, skilled nursing, hospice, or other levels of care. Serve as a trusted advocate for patients and families by connecting them with community resources, financial assistance programs, insurance benefits, and support services. Facilitate critical conversations regarding advance care planning, advance directives, and end-of-life decision-making when appropriate. Evaluate and address patient safety concerns through risk assessments related to abuse, neglect, substance use, homelessness, and other social determinants of health. Partner effectively with physicians, nurses, case managers, and other healthcare professionals to improve care coordination and patient outcomes. Maintain accurate, timely, and compliant documentation within the electronic medical record while meeting organizational and regulatory standards. Contribute to interdisciplinary rounds, case reviews, and quality improvement initiatives that enhance patient care and organizational performance. Uphold ethical, legal, and professional standards while advocating for equitable access to care and services.

Proactively identify barriers to care and develop creative, patient-focused solutions Required Education & Licensure Master's Degree in Social Work (MSW) Current Licensed Clinical Social Worker (LCSW) license in good standing Required Experience Three (3) years of social work or related professional experience Required Knowledge & Skills Knowledge of healthcare regulations, including HIPAA, Joint Commission standards, and Choctaw Nation policies Experience with Electronic Medical Records (EMR/EHR) systems Proficiency with Microsoft Office applications Strong communication, collaboration, and organizational skills Ability to work effectively within interdisciplinary healthcare teams Valid driver's license

Pay:

$63,000.00 - $75,000.00 per year
