This position requires interaction with a variety of patient populations including, but not limited to, neonatal, pediatric, adolescent, adult and geriatric. The Emergency Department Clinical Social Worker 2 WIP position provides direct intervention with patients and families and jointly with the health care team, provides assessment, problem solving, advocacy, community referrals and crisis intervention services. Minimum of one (1) year of clinical social work experience Experience

Clinical social work experience in an acute care, setting Related experience in acute discharge planning, home, health, case management, long term care or related community agency Education/training

Master's of Social Work Degree from a Council on Social Work Education accredited program Education/training

N/A Independent action(s) required: Primary responsibilities include proactive development, implementation and ongoing revision of an interdisciplinary plan of care for patients who receive care in the Emergency Department. Negotiates and collaborates with the interdisciplinary team, community resources and payers to facilitate timely and cost-effective discharge plans that enhance appropriate health and social services in the continuum of care.
