Job Description
Overview Join our dedicated team as a Qualified Developmental Disability Professional (QDDP), where you will play a vital role in supporting individuals with developmental disabilities to achieve their highest potential. This dynamic position offers an opportunity to work closely with clients, families, and interdisciplinary teams to develop personalized care plans, ensure compliance with regulatory standards, and promote community integration. As a QDDP, your expertise will directly impact the quality of life for those you serve, fostering independence and empowerment every day. Responsibilities Conduct comprehensive assessments of individuals' needs, strengths, and goals to inform personalized service plans. Develop, implement, and monitor individualized treatment and support plans aligned with each client's unique needs. Coordinate medical and behavioral health services, including discharge planning and clinical case management, ensuring seamless care transitions. Collaborate with families, caregivers, healthcare providers, and community resources to optimize client outcomes. Maintain accurate documentation within electronic health records (EHR), ensuring compliance with HIPAA regulations and medical terminology standards. Provide ongoing client advocacy by monitoring progress, adjusting care strategies, and supporting behavioral health interventions. Facilitate intake processes and participate in utilization management to ensure appropriate service delivery. Qualifications Valid certification as a Qualified Developmental Disability Professional (QDDP) or equivalent credential. Experience working with individuals with disabilities across various settings such as inpatient, outpatient, or community-based programs. Knowledge of medical terminology, physiology, and clinical assessment techniques relevant to developmental disabilities. Strong understanding of behavioral health principles, mental health counseling, addiction counseling, and working with individuals with substance use disorders. Proficiency in electronic health records (EHR/EHR) management systems and healthcare compliance standards like HIPAA. Excellent communication skills for working effectively with clients from diverse cultural backgrounds and their families. Ability to prioritize tasks efficiently while managing multiple cases in fast-paced environments such as hospital medicine or emergency settings.
Pay:
From $20.00 per hour Application Question(s): QDDP Certification or eligible
