Licensed Social Worker or Mental Health Counselor

Job Description

B & D Integrated Health Services East is seeking a compassionate, motivated, and dedicated Provisionally Licensed or Fully Licensed Social Worker or Mental Health Counselor to join our Intensive In-Home (IIH) team. This position provides community-based mental health services to children, adolescents, and families, including comprehensive assessments, individual and family therapy, case management, crisis support, and treatment planning. We are looking for professionals who possess strong clinical judgment, excellent communication and documentation skills, and a passion for helping youth and families achieve positive outcomes. Responsibilities Conduct comprehensive clinical assessments and ongoing reassessments. Provide individual, family, and therapeutic interventions in home and community settings. Develop, implement, and monitor Person-Centered Plans. Coordinate care with schools, medical providers, community agencies, and natural supports. Conduct Child and Family Team (CFT) meetings. Provide psychoeducation and skill-building services. Monitor client progress and adjust treatment interventions as needed. Assist with crisis intervention and stabilization services. Complete referrals, discharge planning, and required documentation. Maintain compliance with all Medicaid, state, and agency requirements. Participate in on-call responsibilities and required trainings. QualificationsRequired Education Master's Degree in Social Work, Counseling, Psychology, Human Services, or a related behavioral health field. Required Licensure Applicants must hold a valid North Carolina license, including:
LCSWA LCSW LCMHC-A LCMHC
Or another qualifying provisional or independent behavioral health license recognized by North Carolina. Required Experience Minimum of 1 year of experience working with Mental Health and/or Substance Abuse populations. Experience working with children, adolescents, and families preferred. Strong assessment, clinical documentation, interpersonal, and time-management skills. Ability to work independently in community-based settings.
