The Collaborative Care Specialist is a key member of our Collaborative Care Model (CoCM) and works closely with the patient's pediatric medical provider and psychiatric consultant to support the behavioral and physical health needs of children and adolescents. This position primarily supports patients experiencing anxiety, depression ages 12-18, ADHD ages 6-12, and other common behavioral or emotional health concerns . Unlike traditional long-term mental health counseling, Collaborative Care provides brief, focused periods of counseling and behavioral health support , generally lasting 6-12 months , while patients continue to receive care within their pediatric medical home. Duties and Responsibilities Support the behavioral and physical health needs of an assigned caseload of approximately 60-80 patients , with patients generally contacted at least twice monthly. Work closely with pediatric medical providers, the psychiatric consultant, and other mental health professionals to coordinate patient care. Screen and assess patients for common behavioral health concerns and help engage children and families in appropriate treatment and follow-up. Provide education to patients and families regarding common mental health conditions, behavioral concerns, and available treatment options. Systematically track patient progress and treatment response and monitor for changes in symptoms, treatment side effects, or other concerns. Support psychotropic medication management as prescribed by medical providers by monitoring treatment adherence, effectiveness, and potential side effects. Provide brief, evidence-based behavioral interventions , including cognitive behavioral therapy techniques, behavioral activation, problem-solving strategies, motivational interviewing, coping skills, and other appropriate interventions. Facilitate referrals for additional or longer-term behavioral health services when clinically indicated. Participate in weekly caseload consultations with the psychiatric consultant , with particular attention to new patients and patients who are not improving as expected. Document patient contacts, clinical outcomes, progress, and treatment recommendations in the EHR and maintain an accurate patient registry. Follow up with patients and families who have missed appointments or may need additional support remaining engaged in care. Communicate treatment recommendations and patient progress with medical providers, the psychiatric consultant, and other members of the patient's care team. Required Qualifications Current LMSW or LCSW licensure in New York State Strong interest in working with children, adolescents, and families Excellent oral and written communication skills Compassionate, supportive, patient, and family-centered approach Ability to work independently while also being an active member of a collaborative healthcare team Ability to communicate behavioral health needs and interventions clearly and professionally with medical providers Strong organizational and follow-up skills Ability to work effectively with patients and families from diverse backgrounds and cultures Experience in pediatric, behavioral health, primary care, or collaborative care settings is preferred
