Our mission is to enhance the quality of life for individuals and families living with or at risk for HIV/AIDS, Hepatitis C, and other chronic conditions. We are committed to providing comprehensive healthcare and social support services at no cost to our clients. Position Summary The Hook Up and Bridge (HUB) Program is designed to deliver holistic HIV prevention and care services while linking clients to supportive services. The program emphasizes client choice, wellness, and quality service provision, recognizing the importance of supportive services in overall health and well-being. The Mental Health Professional in the HUB Program will provide low-threshold, trauma-informed mental health services to individuals facing barriers to care, including people who use drugs, TGNCNBI+ individuals, and people who exchange sex for money. Working within a harm reduction framework, the Mental Health Professional provides brief mental health interventions, crisis support, referrals, and care coordination to support engagement in HIV prevention, care, and other health services. Services are provided at ACQC's Jamaica and Woodside locations, as well as through community-based and mobile outreach activities. This work is carried out in alignment with the mission and goals of the AIDS Center of Queens County (ACQC), Inc. Key Responsibilities Provide individual counseling, brief interventions, and crisis support. Conduct mental health screenings and assessments. Deliver services using trauma-informed and harm reduction practices. Provide affirming and culturally responsive services to LGBTQ+ individuals. Support clients with substance use concerns without requiring abstinence or participation in treatment. Assist clients with referrals to medical care, substance use treatment, housing, benefits, and other supportive services. Collaborate with outreach, navigation, case management, and behavioral health staff. Maintain accurate and timely documentation in accordance with agency and funder requirements. Participate in team meetings, supervision, and staff trainings. Provide services at multiple program locations and through mobile outreach as needed. Participate in occasional evening and weekend outreach activities. Perform other duties as assigned. Qualifications Required LMSW, LCSW, or LMHC required. Minimum two years of clinical experience in mental health services. Experience working with individuals impacted by substance use and knowledge of harm reduction principles. Experience working with LGBTQ+ communities. Experience providing trauma-informed services. Bilingual English/Spanish required. Strong communication, documentation, and organizational skills. Ability to work independently and as part of a multidisciplinary team. Ability to provide services in office, community, mobile, and telehealth settings. Proficiency in Microsoft Office and electronic record systems. Valid driver's license in good order preferred. Sensitivity to ethnic, cultural, gender, and sexual diversity and ability to provide nonjudgmental, client-centered services. This position is a full-time, exempt position. Primary work location is the Richmond Hill Site, with responsibilities also performed at the Woodside Harm Reduction Site.
