Job Overview We are seeking a dedicated and compassionate Case Manager and Counselor to join our dynamic healthcare team. In this vital role, you will coordinate comprehensive patient care plans, provide addiction counseling, and facilitate effective discharge planning. Your expertise will help ensure patients receive personalized support, access community resources, and achieve optimal health outcomes. This position offers an exciting opportunity to make a meaningful difference in the lives of rural eastern Oregon within a fast-paced, multidisciplinary environment. Duties Conduct patient ASAM assessments to determine appropriate level of care and patient needs that inform the individualized service plans Provide addiction counseling to patients with behavioral health conditions or substance use disorders Coordinate residential and outpatient substance use disorder treatment services, ensuring seamless transitions across the continuum of care Collaborate with community partners, and facilitate discharge planning Engage with patients and their families to deliver client-centered care, education, and support throughout treatment episode An understanding of HIPAA regulations and 42 CFR pt 2, protecting patient confidentiality Connect patients with community resources such as social, legal, employment, recovery services,, and housing supports Requirements Current certification in CADC-I, CADC-II, or CADC-III or must register with Division approved credentialing board within 30 days of hire and certified within one year Knowledge of substance use disorders, recovery principles, and evidence-based treatment practices Ability to conduct ASAM assessments, treatment planning, individual counseling, group therapy, case management, and care coordination within the scope of credential Ability to maintain timely, accurate, and professional clinical documentation Understanding of person-centered, trauma-informed, culturally responsive, and recovery-oriented care Ability to establish strong therapeutic relationships while maintaining appropriate professional boundaries Comfortable working with individuals experiencing substance use disorders, co-occurring mental health conditions, housing instability, justice-system involvement, and other barriers to recovery Strong communication and interpersonal skills and the ability to work collaboratively as part of a multidisciplinary team Commitment to providing respectful, nonjudgmental, low-barrier services and meeting patients where they are Familiarity with ASAM Criteria Fourth Edition preferred Basic computer proficiency Must pass applicable Oregon credentialing and background check Preferred qualifications Experience providing outpatient substance use disorder treatment services, facilitating groups, working with individuals prescribed medications for opioid use disorder (MOUD),support individuals. with co-occurring disorders, or serving rural and underserved populations is valued but not required. Experience working with individuals with mental health conditions or substance use disorders; familiarity with addiction counseling preferred Strong skills in ASAM assessment, task prioritization, client communication, and discharge planning Ability to work effectively with diverse cultural backgrounds and individuals with disabilities or complex social needs Well versed in the Oregon Administrative Rules (OARS) for behavioral health outpatient
Pay:
$22.38 - $38.36 per hour Expected hours: 40.0 per week
