POSITION SUMMARY
Hiring Immediately | $23/hr Starting Pay | Non-Medical Case Management What This Role Is (And What It's Not) What it
IS:
You are the main point of contact making sure our clients, their families, and their Service Coordinators feel taken care of and in the loop. You turn referrals into clear plans, match clients with the right Success Partners (our direct staff), keep services running smoothly, and step in early when something feels off. What it is
NOT:
This is not a medical, clinical, or hands-on caregiving role. You will not provide personal care (toileting, bathing, feeding) or clinical treatment. Your work is communication, coordination, problem-solving, and documentation that keeps everyone confident and happy with services.
Your Day-to-Day Might Include:
Calling new families to welcome them, explain next steps, and schedule in-home assessments
Meeting clients and families in their homes to learn about goals, routines, and support needs
Writing clear assessment reports and recommendations for Regional Center Service Coordinators
Recommending Success Partner matches based on gender, age, energy, and interests, then helping build a schedule that actually works for the family
Pre-framing first sessions so families know what to expect and feel comfortable
Checking in during the first few weeks to see how the match feels and fix issues early
Reviewing daily notes in AxisCare to make sure visits are documented and nothing important slips
Coaching Success Partners on TRA standards for communication, culture, and documentation
Hosting 30-day, semi-annual, annual, and some quarterly meetings with families and Service Coordinators
Keeping simple, clear internal notes so anyone on the team can see where each case stands Perks & Pay

Want to grow long-term in IDD services, case management, or program leadership Minimum Requirements
At least 2 years of experience working with people with intellectual and developmental disabilities (IDD) in a direct support, case management, or coordination role
Strong written and verbal communication skills
Comfortable using email, Google Workspace, and web-based software (we use AxisCare)
Reliable transportation for in-home visits and community meetings within the service area
Able to pass background check and other requirements tied to Regional Center work
Legally authorized to work in the United States Physical Requirements
Ability to travel to client homes and community locations (driving required)
Ability to sit for extended periods for documentation, calls, and virtual meetings
Ability to stand, walk, and move around client homes (including stairs in some locations)
Ability to carry typical work materials (laptop/tablet, bag, paperwork)Schedule
Full-time role with a primarily daytime schedule
Occasional early evening meetings to accommodate families or Service Coordinators
