Friday SUMMARY:

The role of the Case Manager is crucial in supporting clients throughout their substance abuse treatment and helping ensure a successful transition through each stage of recovery. The Case Manager is responsible for supporting and mentoring clients throughout their stay while working closely with the clinical, nursing, and treatment teams. The Case Manager helps identify individual client needs, coordinates services, assists with treatment and discharge planning, and connects clients with appropriate resources for continued recovery. Daily responsibilities will vary and may include completing intake assessments, treatment planning, discharge planning, crisis intervention, one-on-one client sessions, group facilitation, coordinating appointments and outside services, assisting with aftercare placement, and other tasks necessary to support the client's individualized treatment plan. Accurate and timely documentation of case management activities in Kipu will be required.

RESPONSIBILITIES

Complete client intake and case management assessments. Meet individually with clients to discuss treatment goals, progress, concerns, and discharge needs. Assist with the development and implementation of individualized treatment plans. Provide ongoing support, mentoring, and guidance throughout the client's stay. Assist with crisis intervention and communicate safety concerns to appropriate clinical or medical staff. Facilitate educational or recovery-focused groups as assigned. Coordinate appointments, referrals, transportation, and other client needs when appropriate. Assist clients with identifying barriers to continued treatment and recovery. Develop appropriate discharge and aftercare plans. Coordinate referrals to residential treatment, PHP, IOP, outpatient treatment, sober living, and other community resources. Communicate with treatment providers, referral sources, and authorized family members when appropriate. Participate in treatment team meetings and collaborate with nursing, medical, clinical, and support staff. Complete accurate and timely progress notes and case management documentation in Kipu. Maintain professional boundaries while providing compassionate and respectful client support. Maintain client confidentiality and comply with

HIPAA, 42 CFR

Part 2, and facility policies.

REQUIREMENTS

Previous experience working in substance abuse treatment, detoxification, behavioral health, case management, or a related setting preferred. Experience as a Case Manager in a substance abuse treatment environment strongly preferred. Knowledge of substance abuse, addiction, withdrawal, relapse prevention, and the recovery process. Knowledge of treatment planning, discharge planning, crisis intervention, and client documentation. Knowledge of different levels of substance abuse treatment and available community resources. Knowledge of treatment and referral resources within Southern California preferred. Experience working with individuals with substance abuse and co-occurring mental health conditions. Strong interpersonal, communication, organizational, and documentation skills. Experience with Kipu preferred. Ability to work effectively as part of a multidisciplinary treatment team. Current First Aid/CPR certification preferred. High school diploma or equivalent required.

Preferred :

Current certification through CADC, CATC, SUDCC, or an equivalent substance use disorder counselor certification.

Pay:

$25.00 - $28.00 per hour
