Job Description

El Sol NEC's mission is for community members to reach personal empowerment to move towards self-sufficiency and contribute to their community's well-being. The Behavioral Health Specialist at El Sol Neighborhood Educational Center provides essential behavioral health support and care coordination services to Enhanced Care Management (ECM) Members experiencing behavioral health conditions. This role focuses on supporting Members, particularly those with Serious Mental Illness (SMI) and/or Substance Use Disorder (SUD), through member engagement, behavioral health education, motivational interviewing, wellness planning, care coordination, and connection to appropriate community and clinical resources. The Behavioral Health Specialist works collaboratively with licensed providers, the RN Care Manager, primary care providers, behavioral health providers, and other members of the care team to support coordinated, member-centered care Minimum requirements: 2+ years of related/equivalent experience in a healthcare or social services role w/BA or 4+ years of related/equivalent experience in healthcare or social services role with some college; Bilingual (Spanish/ English; speak and write) preferred. Must clear FBI/DOJ background check.
Qualifications:
Minimum of two (2) years of experience working with individuals experiencing behavioral health needs, particularly Serious Mental Illness (SMI) and/or Substance Use Disorder (SUD). Knowledge of evidence-informed engagement and support practices, including Motivational Interviewing and behavioral activation strategies. Ability to provide member-centered support, education, care coordination, and resource navigation within the scope of the position. Ability to work collaboratively as part of a multidisciplinary care team and communicate effectively with Members, families, providers, and community partners. Strong attention to detail and accuracy in data entry, documentation, and record management. Knowledge of local social services and community resources available to support Members' health, behavioral health, and social needs. Experience supporting health education programs, engagement, or community-based services. Ability to understand and respond effectively to the needs of a multicultural community. Strong professional ethics, sound judgment, discretion, and ability to maintain the confidentiality of sensitive information. Ability to perform tasks involving sitting, standing, walking, bending, moving, and occasionally lifting up to 50 pounds
