In this role, you'll make a difference in the lives of people with substance abuse challenges, and often with co-occurring conditions. You'll use and grow your skills in compassionate care, crisis intervention and de-escalation, in a supportive and inclusive work environment. You'll have the opportunity provide our community with substance abuse treatment to help people who are just discovering possibilities in their journey, going from a place of hopelessness to a place where they are valued and have the potential for a better life.
Responsibilities:
Provide case management services to clients at the Detox Center. Schedule and arrange client medical, psychiatric, and or continuing care appointments. Maintain appropriate notation and documentation in client file. Maintain accurate documentation in accordance with case management billing practices. Provide timely crisis intervention when needed. Meet with clients to determine case management and referral needs. Participate in all required consultation with Alameda County Behavioral health providers regarding client care and case management experience and opportunities. Maintain positive working relationships with community providers and managed care organizations. Assist with case management training at staff meetings. Provide clients with educational materials and orientation of continuing care options, including withdrawal management, mental health services, residential treatment, and recovery residences. Demonstrate good decision making on a daily basis. Utilize email and other forms of technology to communicate with peers, supervisors, and community members. Attend all scheduled team meetings and trainings. Assist with data entry in to EHR system as needed. Drive program vehicle when necessary. Perform any other duties as assigned by Program Manager. What will make you successful in this role? Current registration or certification with either Substance Use state certifying body in California (CAADE/CCAPP/CADTP), or registration with the California Board of Behavioral Sciences (BBS) as an AMFT or ASW, preferred Ability to use a computer; MS Word, Excel, PowerPoint, database systems, write reports Experience providing case management services in a mental health or substance use disorder program in the non-profit sector. Knowledge of demographic data collection Safeguard Protected Health Information (PHI) Ability to manage time and prioritize Experience presenting programs and services in the community. Ability to execute projects and measure results/ impact Experience working with customers or clients that feel overwhelmed or in a crisis (crisis intervention) Experienced working with diverse populations and cultures Able to exercise good judgment and maintain healthy boundaries with participants. What else is required? Valid California driver's license with a good driving record and car insurance. Current CPR/First Aid certificate desirable. Salary:
