The ideal candidate will have a strong background in special education, patient care, and behavioral health, with a focus on supporting individuals with developmental disabilities. As a Case Manager, you will play a crucial role in assessing patient needs, developing care plans, and coordinating services to ensure the best outcomes for our clients. Responsibilities Conduct comprehensive patient assessments to identify needs and develop individualized care plans. Collaborate with healthcare providers, families, and community resources to coordinate services. Maintain accurate documentation of patient progress and treatment plans in compliance with HIPAA regulations. Collect and analyze data related to patient care and outcomes. Provide support and guidance to patients and their families regarding available resources and services. Advocate for patients' rights and needs within the healthcare system. Monitor patient progress and adjust care plans as necessary to ensure effective treatment. Stay informed about developments in medical terminology and best practices related to behavioral health. Qualifications
Bachelor's degree or higher in ABA/Psychology/ Social Work or related field
Previous experience working with individuals with developmental disabilities
Provide case management services to individuals with developmental disabilities
Develop and implement individualized service plans
Coordinate with external agencies to ensure comprehensive care for clients
Conduct assessments to identify client needs and goals
Monitor client progress and adjust service plans as needed
Collaborate with a multidisciplinary team to address client needs
