Pineland BHDD is seeking an organized and compassionate Integrated Behavioral Health Case Manager to support individuals receiving coordinated behavioral and physical healthcare services. This role helps connect individuals to the right services at the right time by collaborating with behavioral health providers, primary care professionals, community partners, and support systems to improve health outcomes and overall well-being. Through this role, you will help individuals navigate healthcare systems to improve whole-person care for those we serve. What You'll Do Integrated Care Coordination Coordinate services among behavioral health providers, primary care providers, and support teams Support screening, referral, and follow-up processes for mental health and substance use services Track patient engagement, service utilization, and outcomes Participate in integrated care initiatives designed to improve access and continuity of care Assist with transportation coordination and appointment support when needed Outreach & Community Engagement Conduct outreach activities to increase awareness of available services Support referral development through community events and partnerships Educate individuals and families about available behavioral health and healthcare resources Strengthen relationships with healthcare providers and community organizations Case Management & Individual Support Assist individuals with identifying needs, setting goals, and accessing services Help complete necessary paperwork and documentation Link individuals to community resources and ongoing support services Participate in multidisciplinary team meetings and care coordination activities Support individuals with complex behavioral health and medical needs through a person-centered approach Documentation & Compliance Maintain accurate, timely, and audit-ready records Complete required progress notes, reports, and service documentation Ensure service authorizations and documentation are completed according to agency requirements Maintain compliance with HIPAA, payer requirements, and organizational standards Utilize electronic health records and care management systems effectively Preferred Education Associate's or Bachelor's Degree in: Healthcare Administration, Social Work, Psychology, Public Health, or a related field Additional training in integrated care, care coordination, or population health is a plus. Preferred Experience 1-4 years of experience in: Healthcare coordination Behavioral health services Care management Healthcare administration Integrated healthcare settings Or a related field Skills That Lead to Success Knowledge of integrated behavioral health and primary care models Strong care coordination and resource linkage abilities Excellent communication and relationship-building skills Attention to detail and strong documentation practices Ability to work effectively with multidisciplinary teams Proficiency with: Electronic Health Records (EHR) Microsoft Office Suite Microsoft Teams Email and data management systems Strong customer service and organizational skills Additional Information Community-based position with local travel required Requires collaboration with internal and external healthcare providers Must maintain confidentiality and HIPAA compliance Participation in ongoing training and professional development required Pineland BHDD is proud to be an Equal Employment Opportunity (EEO) Employer and maintains a drug-free workplace.
