Hours:

Days What you will do: Conduct foundational psychosocial assessments and gather collateral information to support development of treatment and discharge plans.

Implement treatment and discharge plans in collaboration with the interdisciplinary team.

Provide crisis intervention and supportive therapeutic interactions (individual, group, family) under guidance of senior staff.

Coordinate discharge planning activities, including scheduling follow-up appointments and initiating standard referrals.

Identify patient and family needs (emotional, social, financial) and escalate complex or high-risk barriers to senior case management staff.

Maintain accurate, timely clinical documentation in accordance with regulatory and organizational standards What you will need: Required Education, Skills and/or

Experience:

Master's Degree in Social work or a Master's Degree in a recognized mental health field

1 year of experience in behavioral health environment preferred If unlicensed, must be eligible to sit for licensing exam within the next 6 months as applicable. Must have obtained Licensed Professional Counselor (LPC) or Licensed Social Worker (LSW) or Licensed Marriage Family Therapist (LMFT) licensure within one year of start date. Preferred

•Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT). Current CPR/BLS Certification issued by American Heart Association within 90 days of hire
