We are seeking a dedicated Case Manager to join our Residential Treatment Program and make a meaningful difference in the lives of youth. Position Summary The Case Manager is responsible for coordinating services and maintaining a residential caseload of up to twelve youth. This position ensures timely documentation, facilitates communication among treatment team members, families, and state agencies, and advocates for each child's individualized treatment needs throughout their stay. Essential Responsibilities Maintain a caseload of up to 12 residential youth. Conduct comprehensive intake assessments by gathering historical and current information to identify each child's individualized treatment needs. Ensure all documentation is completed accurately, distributed appropriately, and maintained in accordance with agency and regulatory standards. Serve as the primary point of communication with families, DCBS workers, treatment providers, schools, and other stakeholders. Maintain organized electronic case records with strong attention to detail and compliance. Complete and submit required documentation within established deadlines, including: ALPs submitted to the Children's Review Program (CRP) Monthly Progress Reports (1294 A/B) PCC Tracking/TWIST documentation Meet face-to-face with each assigned resident at least twice weekly to monitor progress, address concerns, and communicate updates with the treatment team. Participate in treatment planning and collaborate with clinical, medical, educational, and residential staff. Coordinate discharge planning to ensure successful transitions to home, foster care, or other appropriate placements. Maintain professionalism, confidentiality, and integrity while advocating for the best interests of each child. Perform other duties as assigned to support the Residential Treatment Program. Qualifications Required Bachelor's degree in Social Work, Psychology, Human Services, Sociology, Counseling, or a related human services field. Strong written, verbal, and interpersonal communication skills. Excellent organizational and time management skills. Ability to work independently while collaborating effectively within a multidisciplinary team. Proficiency in Microsoft Office (Word, Excel, Outlook). Ability to prioritize multiple responsibilities and consistently meet deadlines. Commitment to maintaining confidentiality and professional ethics. Valid driver's license with an acceptable driving record. Preferred Experience working with children, adolescents, or families. Experience in residential treatment, foster care, behavioral health, or child welfare. Knowledge of Kentucky DCBS and behavioral health documentation systems is a plus. Physical Requirements Regular walking, standing, and bending. Occasionally lift, carry, push, or pull up to 25 pounds. Regular operation of a motor vehicle. Work Environment Work is performed in both office and residential treatment settings. Employees may occasionally be exposed to contagious illnesses and behavioral crisis situations while supporting youth in care.
