We are a fast-growing private behavioral health practice seeking a Targeted Case Manager (TCM) to join our supportive, client-centered team. This role is ideal for someone who is passionate about helping individuals with mental health needs access the care, resources, and support they deserve. As a TCM, you'll provide case management services to individuals diagnosed with behavioral health conditions, assisting with goal setting, care coordination, and connection to essential community services.
Responsibilities:
Provide community-based behavioral health case management to eligible adult clients Complete assessments and develop individualized service plans (ISPs) Coordinate services with mental health professionals, healthcare providers, and social service agencies Assist clients in accessing housing, education, employment, and other support services Assist clients in applying for and resolving lapses in KY Medicaid/MCO coverage Maintain accurate, timely documentation in accordance with Medicaid and state guidelines Monitor and evaluate client progress, adjusting plans as needed Participate in team meetings, supervision, and training
Qualifications:
Bachelor's degree in Social Work, Psychology, Sociology, or a related human services field (required) Must be eligible for or hold current Targeted Case Management (TCM) certification in Kentucky (required) Minimum of 1 year of TCM experience working with adults with SMI diagnoses required Reliable transportation and valid driver's license (required for community-based visits) Strong communication, documentation, and organizational skills Ability to work independently and manage time effectively
