Anchored Hope Counseling & Consulting is seeking a qualified, compassionate, and motivated Targeted Case Manager (TCM) to join our behavioral health team in Pikeville, Kentucky, serving individuals and families throughout Pike County and the surrounding areas. RESPONSIBILITIES

The Targeted Case Manager will work directly with clients to identify needs, coordinate services, connect individuals with community resources, and support progress toward greater stability and independence.

Responsibilities include:

Monitor, assess, and document clients' needs, progress, and barriers to care. Complete comprehensive client needs assessments. Assist in developing individualized and measurable case management goals. Provide ongoing support to clients experiencing behavioral health, social, financial, medical, housing, transportation, employment, educational, and other needs. Connect clients with appropriate state, local, and community resources. Coordinate services with behavioral health providers, medical providers, social service agencies, schools, and other community organizations. Advocate for clients and assist them in navigating healthcare, benefits, and community service systems. Monitor client progress and identify changes in needs or circumstances. Maintain accurate, complete, and timely electronic documentation. Participate in treatment coordination and interdisciplinary team consultation as appropriate. Maintain professional boundaries and protect client confidentiality. Comply with HIPAA, Medicaid, agency policies, and applicable state and federal requirements.

EDUCATION REQUIREMENT

Applicants must: Hold a Bachelor of Arts (BA) or Bachelor of Science (BS) degree in a social or behavioral science field from an accredited institution. Qualifying fields may include psychology, social work, sociology, nursing, counseling, human services, or another applicable social or behavioral science field. Meet applicable Kentucky requirements for providing Targeted Case Management services.

QUALIFICATIONS & SKILLS

The ideal candidate will demonstrate: Excellent professional verbal and written communication skills. Strong computer and electronic documentation skills necessary to perform essential job duties. Excellent organizational and time-management skills. Ability to work independently and effectively manage a caseload. Working knowledge of state and community resources available to individuals and families. Strong interpersonal, advocacy, and presentation skills. Ability to communicate and collaborate professionally with clients, families, healthcare providers, and community agencies. Knowledge of treatment planning is preferred. Previous experience in behavioral health, social services, case management, or a related field is preferred. Ability to maintain appropriate professional boundaries while providing compassionate, client-centered services. Ability to maintain client confidentiality and appropriately handle protected health information.

TRANSPORTATION REQUIREMENTS

Because Targeted Case Management may involve providing services within the community, applicants must: Maintain a valid driver's license. Have access to a dependable and reliable automobile. Maintain automobile insurance that meets applicable Kentucky requirements. Be willing and able to travel throughout Pike County and surrounding service areas as necessary to meet client needs.
