SUMMARY OF RESPONSIBILITIES

Will provide core elements of case management to include assessment, service plan development, linking/coordination of services, reassessment/follow-up, and monitoring of services. Focuses on bolstering parenting skills, enhancing knowledge of child development, supporting positive parent-child interaction, resolving emotional conflicts, and assisting with the management of stressful daily events. Will provide clinical support services to parents and children with emotional or behavioral problems. Additionally, this professional will be responsible for monitoring referrals to internal or external providers or community resources. The Case Manager will also be responsible for working with contracted providers to ensure that person-centered services are being provided. This includes but is not limited to the review of graphing, coordination with the contractor and the family, and overall monitoring of progress.

QUALIFICATIONS

Education:

Bachelor's degree required from a recognized or accredited institution, with coursework in mental health-related areas. Must attain the highest level of licensure according to state guidelines.

Experience:

One or more years of relevant mental health experience with Autism Spectrum Disorder (ASD) experience preferred. Lived experience with mental illness, intellectual/developmental disabilities, and/or substance use disorders valued.

Other:

Good communication skills (both verbal and written) are required. Functional computer literacy is required. Must have a Michigan driver's license and a satisfactory driving record as determined by the agency.

ESSENTIAL JOB FUNCTIONS

Demonstrates understanding of assessment and diagnosis for persons requesting Mental Health Services in accordance to the latest edition of the DSM and ICD. Demonstrate effective case recording in accordance with Person Centered Planning, and NeMCMHA "Case Recording Guidelines". Consults with various agencies, educational settings, individuals, and organizations as needed. Will coordinate with contract providers to ensure individuals served are progressing and act in accordance with current procedures. Fluency in graphing for services provided to the Autism population. Will participate in the agency's Mobile Intensive Crisis Stabilization Services (ICSS) on-call rotation. Will serve as an active member of the Children's Diagnostic and Treatment Team. Demonstrates knowledge of universal precautions, standards of practice, including but not limited to confidentiality and recipient rights, and emergency protocol.

NON-ESSENTIAL JOB FUNCTIONS

Performs other related duties as required. May be requested to be involved in the Board's continuous quality improvement program. May be required to provide own transportation while performing agency-related duties.
