ACT Practitioner Job Summary We are seeking a compassionate and motivated ACT Practitioner to join our dynamic mental health team. In this role, you will provide comprehensive Assertive Community Treatment (ACT) services to individuals experiencing severe and persistent mental health challenges. The ACT Practitioner works closely with clients in community-based settings to support recovery, increase independence, and improve overall quality of life. This position involves collaboration with a multidisciplinary team to provide individualized care, therapeutic support, and crisis intervention services that promote long-term stability and resilience. Responsibilities Provide individualized, evidence-based mental health services in community settings, meeting clients where they are most comfortable. Conduct comprehensive in-person assessments on a regular basis. Collaborate with a multidisciplinary treatment team to coordinate and deliver integrated client care. Support clients in their recovery journey through motivational interviewing, psychoeducation, skill-building, and therapeutic interventions. Monitor client progress and adjust treatment approaches as needed to support positive outcomes. Maintain accurate and timely documentation including treatment plans, progress notes, and service records. Assist clients with accessing community resources and developing independent living skills. Participate in team meetings, case consultations, and ongoing treatment planning. Qualifications Bachelor's degree in Social Work, Psychology, Human Services, or a related field required. Previous experience in community mental health or working with individuals with severe and persistent mental illness preferred. Knowledge of psychopharmacology principles and medication management strategies preferred. Strong interpersonal, communication, and relationship-building skills. Ability to work independently while also collaborating effectively within a team environment. Strong organizational skills and ability to manage multiple priorities in a fast-paced setting. Valid driver's license and ability to travel within the community as needed. Why Join Our Team? $24.00 - $28.00 per hour
