See Pay Scale Job Summary Works as part of a multidisciplinary primary care treatment team providing integrated behavioral health services at the STLC Hickory location to support whole-person care for Senior TLC participants. Provides behavioral health and substance use screenings, brief interventions, patient education, referrals, and consultation services within the primary care setting. Collaborates with primary care providers and the interdisciplinary treatment team to assess participants' behavioral, psychosocial, and chronic health needs and develops individualized treatment plans to improve overall health outcomes. Maintains appropriate clinical documentation, coordinates care across disciplines, and promotes integrated, patient-centered care. Position reports directly to the Behavioral Health Lead.

Specifications Education :

Master s Degree required.

Maintain adherence to professional Code of Ethics.
Provide evidence-based assessment, intervention, consultation, and referral for behavioral and chronic health conditions.
Collaborate as an integral member of the interdisciplinary healthcare team to improve patient care and efficiency.
Works collaboratively as part of an interdisciplinary treatment team sharing relevant clinical information and developing specific, individualized treatment goals and interventions.
Develop, implement, and facilitate behavioral health, substance use, and chronic disease care plans using appropriate treatment modalities and community resources.
Support primary care providers with screening, identification, diagnosis, and management of behavioral health, substance use, and chronic disease concerns.
Participate in ongoing assessment and quality improvement efforts related to behavioral health integration.
Provide consultative services and training support to care team members as appropriate.
Utilizes the electronic medical record (EMR) to document participant encounters, track interventions, and ensure accurate and timely clinical documentation in accordance with organizational and regulatory standards.
Uses Microsoft Office applications, including Microsoft Word, to create and maintain reports, correspondence, and other required documentation to support participant care and departmental operations.
Engage in continuing education to maintain licensure and clinical competency.
Works with professionalism and respect to all participants, families, colleagues, supervisors, and visitors of Senior TLC.
