Job Description

A community service provider in Keene, NH is seeking a compassionate and organized Case Manager to join its Child & Family Team. This role provides community-based case management and care coordination to children and families experiencing behavioral health challenges and complex psychosocial needs. Key Responsibilities Provide case management and care coordination services to children, adolescents, and families. Connect clients and families with behavioral health, medical, educational, housing, financial, transportation, and community resources. Coordinate care with hospitals, schools, healthcare providers, residential programs, and community agencies. Assist families with benefits, applications, transportation, and other resource needs. Support clients with treatment goals, daily living skills, coping strategies, and community functioning. Monitor client needs and respond appropriately to clinical concerns and crises. Participate in treatment team meetings, supervision, and case consultations. Maintain timely and accurate documentation in the electronic medical record. Advocate for clients and families while promoting independence, self-determination, and recovery. Qualifications Bachelor's degree in human services or related field. At least one year of experience working with children, adolescents, and families. Strong skills in advocacy, outreach, assessment, crisis intervention, and supportive counseling. Excellent communication, organization, and interpersonal skills. Preferred Experience Community mental health or targeted case management. Residential or psychosocial rehabilitation services. Medicaid documentation and Targeted Case Management. Care coordination with hospitals and community providers. Trauma-informed and recovery-oriented care.
