Mental Health Case Manager / Primary Case Coordinator & Group Facilitator Unity Place of Monmouth County - Oceanport, NJ Full-Time | Monday-Friday | 8:30 AM-4:30

PM Salary:

Starting at $42,000/year; negotiable based on experience and qualifications Make a Difference in Behavioral Health Unity Place of Monmouth County is seeking a compassionate, organized, and motivated Mental Health Case Manager / Primary Case Coordinator to join our multidisciplinary behavioral health team. This hands-on position combines case management, care coordination, group facilitation, and client support. You'll manage an assigned caseload while helping adults receiving behavioral health services work toward greater stability, wellness, and independence. Bachelor's degree required. No master's degree or clinical license required. What You'll Do Maintain an assigned caseload and build supportive, professional relationships with clients Facilitate multiple specialized psychoeducational and therapeutic groups each day within your scope of practice Develop creative and engaging lesson plans, group activities, and educational materials Develop, monitor, and update individualized treatment and service plans Coordinate mental health, medical, psychiatric, and community-based services Make appropriate referrals and connect clients with needed resources Communicate client needs, concerns, and progress with the multidisciplinary treatment team Provide client support and crisis intervention as appropriate Complete daily documentation, weekly progress notes, group notes, and required client records accurately and on time Maintain organized, compliant, and up-to-date client charts, event sheets, and treatment documentation Assist with required quality assurance and program data Follow agency policies, ethical standards, and applicable regulatory requirements What We're Looking For BA/BS in Psychology, Social Work, Human Services, Counseling, Sociology, or a related field Exceptional organization and time-management skills with the ability to balance a caseload, multiple daily groups, lesson planning, and documentation deadlines Strong written, verbal, and interpersonal communication skills Confidence facilitating groups and engaging diverse client populations Strong attention to detail and commitment to timely, accurate documentation Compassionate, dependable, professional, and team-oriented Experience in behavioral health, mental health, case management, social services, or group facilitation is preferred but not required Why Join Unity Place?
