Case Manager Old Bethpage, NY Full-Time Salary:

FREE provides a diverse array of supports and services including: housing; recovery services; transition to work; employment; day, community and family services; respite; crisis services; education and after-school support; specialty health services; and advocacy. Under the supervision of the Critical Time Intervention Team Leader, the Critical Time Intervention (CTI) Case Manager is responsible for providing time-limited evidence-based practice of specialized case management interventions to society's most vulnerable individuals during times of transition. To facilitate community integration and continuity of care, the CTI Case Manager blends the delivery of intensive supports that meet immediate client needs with assistance aimed at facilitating long-term community integration and continuity of care. In addition, you will: Conduct comprehensive client assessments and develop individualized care plans to promote recovery, independent living, and continuity of care. Connect clients with medical, behavioral health, community, and social support services, including benefits, treatment programs, and informal support networks. Provide crisis intervention, screening, referrals, and emergency response while reducing hospitalizations, emergency department visits, and crisis service utilization. Educate clients, families, and support systems on risk prevention, medication adherence, harm reduction, relapse prevention, and recovery resources. Advocate for clients by supporting informed treatment decisions and ensuring compliance with care plans and recovery goals. Collaborate with multidisciplinary teams, psychiatrists, hospitals, CPEPs, emergency departments, and community partners to coordinate seamless client care. Maintain accurate, timely, and compliant documentation, data collection, and case records in accordance with agency policies and CTI best practices. Participate in ongoing professional development, uphold safety and abuse reporting standards, and support agency policies, procedures, and quality improvement initiatives.
We require:
Associate's degree in Human Services (will accept Criminal Justice degrees) At least 1 year of experience supporting individuals with intellectual/developmental disabilities and/or mental health diagnoses preferred Valid driver's license with clean driving record Ability to obtain required CTI certification (within 12 months) and 9.58 designation certification (within 6 months of hire) Knowledge of OMH and OPWDD regulations; proficiency with Microsoft Office and basic computer applications Strong communication, organization, teamwork, and coaching skills with the ability to work collaboratively in a fast-paced environment Ability to meet required health screenings, physical examination, and immunization requirements CTI Case Manager, for immediate consideration, please apply now!
