Education Required:

Bachelor's degree in Social Work or a related field and three (3) years of Case management experience w/ Mental Health and Co-Occurring Disorders population. Masters in Social Work preferred.

Bilingual / Spanish

speaking is ideal. Knowledge of Forensic Services a plus. CADC or CAADC certifications a plus!

Primary Responsibilities:

Some of the primary responsibilities for the Case Manager are as follows:

• Assigned a caseload and interviews respective clients, gathers all pertinent information including previous or present treatment providers, and any criminal justice agencies, etc., provides individual and family education, maintains accurate and complete clinical records, and implements the discharge and referral process.
• Member of the interdisciplinary treatment team. They bring information to the team, have input in the treatment and discharge planning, implements their part of the treatment programming and implements the discharge process.
• Completes required paperwork for each assigned patient, such as Psychosocial history & Assessment, Confidential Release forms, Treatment Plan, Aftercare Plan, Educational Interventions, progress notes and other reports.
• Must have excellent oral and written communication skills, as well as be detail-oriented and able to complete a high volume of documentation in a fast-paced environment.
