This position is dedicated to empowering clients to make healthy, informed decisions while addressing practical, emotional, social, and environmental factors that influence long-term recovery and stability. The Case Manager provides individualized case management services and psycho-educational group facilitation to promote client growth and independence. Corner Canyon serves clients with mental health and substance use concerns. Responsibilities Conduct comprehensive assessments to identify clients' needs, strengths, and goals. Develop personalized case management plans tailored to each client's unique circumstances. Support clients in developing life skills necessary for independent living and community reintegration. Facilitate psycho-educational groups focused on topics such as recovery strategies, life skills, and emotional well-being. Assist clients with future planning, including housing, employment, education, and other essential services. Coordinate with community resources and service providers to ensure clients receive appropriate support. Monitor client progress regularly and adjust plans as needed to promote sustained recovery. Advocate for clients' needs within the clinical, medical, and programming systems. Maintain accurate documentation of client interactions, progress notes, and case management activities in compliance with organizational policies. Requirements Proven experience in case management or related roles within behavioral health, social services, or community support settings. Strong understanding of recovery processes (mental health and substance use), life skills development, and community reintegration strategies. Excellent communication and interpersonal skills to build trusting relationships with clients and stakeholders. Ability to facilitate psycho-educational groups effectively. Trauma informed approach with a commitment to supporting individuals through their recovery journey (mental health and substance use). Organizational skills to manage multiple cases simultaneously while maintaining attention to detail. Knowledge of local resources, social services systems, and relevant regulations is preferred. Must pass a background screening through the Department of Health and Human Services, required.

Pay:

$22.00 - $26.00 per hour Expected hours: 30.0 - 40.0 per week
