Social Services Assistant / Transportation Coordinator Marin Post Acute 📍 San Rafael, CA 💵 $18-$25 per Hour Marin Post Acute is seeking a compassionate, organized, and dependable Social Services Assistant / Transportation Coordinator to join our team. This unique role combines resident advocacy with appointment coordination, helping ensure our residents receive the support and transportation they need to thrive. • Coordinate and schedule resident transportation for medical appointments, therapies, and outside services
• Communicate with families, healthcare providers, hospitals, and transportation vendors
• Assist the Social Services department with resident needs, admissions, discharges, and care planning support
• Help residents access community resources and services as appropriate
• Maintain accurate records, schedules, and documentation
• Provide excellent customer service to residents, families, and visitors
• Serve as a positive resource and advocate for residents throughout their stay
• Ensure transportation arrangements are completed efficiently and on time What We're Looking For
• Previous experience in healthcare, social services, customer service, admissions, or case management preferred
• Strong organizational and time-management skills
• Ability to coordinate multiple appointments and schedules simultaneously
• Excellent communication and interpersonal skills
• Comfortable working with seniors and their families
• Computer proficiency and experience with documentation systems preferred
• Professional, dependable, and compassionate attitude
