We're looking for a compassionate, organized, and knowledgeable Resource Coordinator to join our team and make a meaningful impact in the lives of the individuals and families we serve. What You'll Do Guide & Support
Help individuals, especially those on the autism spectrum and other developmental disabilities, understand and access essential benefits and resources such as Social Security (SSI/SSDI), Medi-Cal, IHSS, CalFresh, housing programs, transportation services, and other community supports. Advocate
Work alongside clients and their families to ensure they receive the services, benefits, and resources for which they may qualify. Assist clients in navigating complex systems, overcoming barriers, and communicating effectively with agencies and service providers. Employment & Education Assistance
Support clients in finding meaningful employment, completing job applications, developing resumes, preparing for interviews, receiving on-site job coaching, communicating with employers, exploring vocational training, and accessing post-secondary education. Help clients develop the skills and confidence necessary for long-term success. In-Home & Daily Living Support
Assist individuals in developing greater independence through organization, maintaining routines, completing household responsibilities, managing appointments, and building other practical daily living skills while working collaboratively with their families. Monitor Client Goals & Progress
Regularly track each client's progress toward their individualized goals and objectives. Maintain accurate documentation of services provided, progress made, challenges encountered, and areas requiring continued support. Prepare Progress Reports
Compile and analyze client progress information to prepare clear, accurate, and professional quarterly, semi-annual, annual, and other required progress reports . Reports should accurately reflect the client's progress toward their established goals, services provided, current needs, and recommendations for continued support. Coordinate With North Bay Regional Center
Communicate and collaborate with North Bay Regional Center Service Coordinators regarding client progress, services, goals, upcoming meetings, reporting requirements, and changes in client needs. Participate in the coordination of Individual Program Plan (IPP/ISP) meetings and provide requested documentation or progress updates in a timely manner. Maintain Accurate Records & Documentation
Keep client files, schedules, case documentation, reports, and other required records organized and current. Ensure documentation is completed accurately, professionally, and within established deadlines. Connect Clients to the Right Resources
Develop and maintain relationships with local agencies, community organizations, employers, schools, healthcare providers, government programs, and other service providers to ensure clients have access to available supports and opportunities. Stay Engaged
Maintain consistent communication and regular meetings with assigned clients to monitor progress, address challenges, identify changing needs, and adjust support strategies when appropriate. Manage Your Caseload & Schedule
Organize multiple client schedules, appointments, reporting deadlines, meetings, and follow-up responsibilities while maintaining consistent communication with clients, families, supervisors, and outside agencies.
Who You Are Experienced in working with individuals with developmental disabilities, particularly individuals on the autism spectrum. Knowledgeable about benefits programs, community resources, and disability services. Highly organized and detail-oriented, with the ability to manage multiple clients, deadlines, appointments, reports, and competing priorities. Comfortable collecting and reviewing information about client progress and translating that information into clear and professional written reports. A strong written and verbal communicator who can effectively communicate with clients, families, Regional Center Service Coordinators, employers, schools, government agencies, and community providers. Able to work independently while maintaining consistent communication with supervisors and other members of the team. Comfortable using technology for documentation, scheduling, communication, and case management. Passionate about helping individuals develop greater independence and achieve meaningful personal, educational, and employment goals. Technology & Administrative Skills Candidates should be comfortable using computers and cloud-based systems as part of their daily responsibilities. Familiarity with Google Workspace (Google Suite) is strongly preferred, including: Google Docs for preparing reports and client documentation. Google Sheets for tracking client information, services, deadlines, and progress. Google Calendar for managing client appointments, meetings, and reporting deadlines. Gmail or similar email platforms for professional communication with clients, families, Regional Center Service Coordinators, and community providers. Google Drive for organizing and maintaining electronic records and shared documents. Candidates should also be able to learn and use additional electronic case-management and documentation systems as required. What You Need 1+ years of experience in disability services, advocacy, case management, resource coordination, social services, or a related field. A background in Social Work, Human Services, Human Development, Psychology, Sociology, Special Education, or a related field preferred. Strong written communication and documentation skills. Strong organizational and time-management skills. Ability to monitor multiple client goals and reporting deadlines simultaneously. Basic proficiency with Google Workspace or similar office productivity software. Reliable transportation. This position requires regular travel throughout Sonoma County, including the City of Sonoma. Empathy, patience, professionalism, and a commitment to helping individuals achieve greater independence. Bonus Points If You Are familiar with Sonoma County-specific disability and community resources. Have experience working with a Regional Center or Regional Center-funded services. Have experience preparing client progress reports, service documentation, case notes, or individualized service plans. Have experience navigating Social Security, Medi-Cal, IHSS, CalFresh, housing programs, or other government agencies. Are highly proficient with Google Docs, Google Sheets, Google Calendar, and Google Drive. Speak Spanish. This is not required, but is a plus.
