As a Care Coordination Outreach Worker , you'll help patients stay connected to the healthcare services they need by completing health risk screenings, coordinating follow-up care, providing education, and conducting outreach in clinic, hospital, and community settings. Position Overview Under the supervision of the Care Coordination Outreach Supervisor , the Care Coordination Outreach Worker supports the delivery and coordination of healthcare services for Aunt Martha's patients. This position completes health risk assessments, conducts patient outreach, schedules follow-up appointments for patients recently discharged from the emergency department or inpatient care, and provides ongoing education related to preventive services and appropriate healthcare utilization. The role works closely with health plans, specialists, hospitals, physicians, and Aunt Martha's clinical teams to support continuity of care. The primary work locations are Park Forest and the Chicago Heights Family Health Center , with travel to Humboldt Park approximately once or twice per month . Key Responsibilities Patient Outreach & Follow-Up Complete health risk screenings for active care coordination patients. Conduct outreach to patients through clinic visits, home visits, and hospital visits as required. Schedule follow-up appointments for patients recently discharged from the emergency department or inpatient care. Help patients stay connected to appropriate medical services and follow-up care. Provide ongoing health education related to preventive services and appropriate emergency department utilization. Care Coordination & Collaboration Collaborate with health plans, specialists, hospitals, physicians, clinic staff, and other members of the care team. Support coordinated care and help address barriers that may prevent patients from receiving needed services. Communicate professionally with patients, healthcare providers, and internal staff. Work independently while contributing to a collaborative Care Coordination team. Documentation & Administrative Support Accurately document patient information and outreach activity in the Athena Electronic Health Record (EHR) . Maintain databases and outcomes information as required by the program. Answer designated telephone calls professionally, respond to inquiries, route communications, and document messages. Maintain accurate and timely records while protecting patient confidentiality. Attend required meetings, trainings, and Core Curriculum sessions. Qualifications Required High School Diploma or equivalent. Experience working with Microsoft Office programs. Strong computer and data-entry skills. Strong verbal and written communication skills. Ability to manage multiple priorities and adapt to changing needs. Strong organizational skills and attention to detail. Ability to work independently and as part of a collaborative team. Current driver's license. Current automobile insurance. Preferred Spanish-speaking / bilingual English-Spanish preferred. Previous experience in healthcare, patient outreach, community services, care coordination, or customer service. Experience using an Electronic Health Record system. Experience interacting with patients, healthcare providers, hospitals, or health plans. Additional Requirements Ability to work primarily from Park Forest and Chicago Heights Family Health Center . Ability and willingness to travel to Humboldt Park approximately 1-2 times per month . Ability to work late evenings, weekends, and scheduled holidays as needed by the program. Ability to perform outreach through clinic, home, and hospital visits as required. Ability to maintain patient confidentiality and follow organizational policies and procedures.
