full-time Targeted Case Manager/Support Coordinator Warren County Developmental Disabilities Board ???? Warrenton, Missouri The Warren County Developmental Disabilities Board is seeking a qualified and compassionate Support Coordinator to support individuals with developmental disabilities and their families. Position Summary The Support Coordinator works collaboratively with individuals, families, and service providers to develop, coordinate, and monitor individualized supports and services. This role ensures services are person-centered, compliant with state and federal requirements, and responsive to individual needs. Essential Job Functions Develop and monitor person centered support plans Advocate for individuals to obtain needed services and supports Coordinate and monitor the delivery of services Maintain accurate documentation in compliance with state and federal regulations Assist in the development of new services and supports as needed Collaborate with families, providers, and community agencies/providers Maintain flexible work hours to accommodate the schedules of individuals and families served The job is an office-based position Minimum Qualifications Bachelor's degree in social work, psychology, special education, or a related field from an accredited four-year college or university At least two (2) years of professional experience assessing service/support needs and developing or implementing individualized support plans for persons with developmental disabilities Preferred Qualifications Three (3) or more years of experience working within the Missouri developmental disabilities waiver services system Bilingual (Spanish/English) applicants Additional Requirements Reliable transportation available daily (position requires travel to homes and agency sites) Proficiency in using a personal computer and standard office software Ability to use typical office equipment Work Location 210 E.
