Position Overview We are seeking a dedicated and compassionate Social Services Assistant to join our interdisciplinary team full-time. In this role, you will help support our residents' psychosocial, emotional, and social needs to ensure they receive holistic, high-quality care throughout their stay. Responsibilities Assist in identifying residents' psychosocial, mental, and emotional needs. Complete social histories and psychosocial assessments for new and current residents. Collaborate closely with the interdisciplinary clinical team to advocate for and protect residents' rights. Help maintain a safe, respectful environment by actively preventing and addressing resident concerns or grievances in accordance with facility policies and mandated reporting guidelines. Assist residents and their families with transition planning, community resources, and daily care adjustments. Complete required documentation accurately within the Electronic Medical Record (EMR) system. Perform other related administrative and supportive duties as assigned by the Director of Social Services or Facility Administrator.
Qualifications Education:
High School Diploma or equivalent required.
Experience:
Experience working with the geriatric population in a long-term care, skilled nursing, or healthcare environment strongly preferred.
Customer Relations:
Strong customer service background with a proven track record of positive resident, family, and community relations preferred.
Technical Skills:
Proficiency with computers and electronic medical record (EMR) systems.
Core Competencies:
Excellent communication and interpersonal skills with the ability to build rapport quickly with residents, families, and team members. Strong organizational skills and the ability to work effectively both independently and as part of a collaborative team.
