Job Description
The Community Liaison builds and maintains relationships in the local community with facilities, families, and individuals by providing continuous education and outreach on the high-quality hospice care that we pride ourselves in providing. This role also includes ensuring that all patients are receiving the highest quality of care possible and providing feedback to management on any issues or opportunities to improve care.
Core Responsibilities:
Finding and building strong relationships with new referral sources including home, health, group homes, hospitals, assisted living facilities, private homes, and skilled nursing facilities. Maintaining quality relationships with existing referral sources through continuous education on Uplift Hospice and the high-quality care we pride ourselves in providing. Communicating and working seamlessly with all team members to provide the best outcomes for our patients and the company. Approaching every day and each situation in a manner that maintains our team culture focused on employee satisfaction, high quality care, and family-orientated environment. Maintaining a high level of creativity and desire to innovate new ways to market and attract referral sources. Comfortability in voicing your opinion as we are always looking to adapt and improve our model of care and team environment. Feedback and transparency are core pillars of our culture. Reporting on a regular basis to the Director of Business Development/Director of Marketing. Periodically making recommendations for changes to our community outreach approach as needed. Participates in developing objectives for Hospice related to community outreach. Observes confidentiality and safeguards all patient related information. Accepts responsibility for regular attendance and punctuality. Investigates and reports any problem relating to patient care or conditions which harm the patient and/or employee well-being. Immediately reports any accident, incident, lost articles or unusual occurrence to the Director of Business Development/Director of Marketing. Attends pertinent continuing education programs other than routine in services and shares information with our team members. Follows up on new patient referrals by communicating with referral sources, meeting with patients and families to fully explain hospice services, help with discharge planning and placement when needed and does care coordination with the intake and clinical teams.
Basic Qualifications/Requirements:
Experience with hospice marketing preferred but not required.
Experience with hospice eligibility criteria, CMS regulations, and clinical documentation preferred.
Ability to cultivate relationships across clinical and operational groups.
Valid driver's license and reliable transportation.
