The Peer Support Specialist (PSS) is an active member the treatment/program team. PSS provides peer support services to members with serious mental illnesses, substance abuse issues and co-occurring disorders under the supervision of the assigned supervisor. PSS will function as a role model to peers; exhibiting competency in personal recovery and use of coping skills; serve as a consumer advocate, providing consumer information and peer support for members in an outpatient setting. The PS performs a wide range of tasks to assist peers of all ages, from young adult to old age, in regaining independence within the community and mastery over their own recovery process. The PSS in the provision of services will utilize recovery resources such as booklets, tapes, pamphlets, and other written materials.
PRINCIPAL DUTIES/RESPONSIBILITIES
Responsibilities:
Focus on identifying Social Determinants of Health needs of members. Resource specialist to resolve Social Determinants of Health. Provides pre-engagement, engagement, and re-engagement of participants, through telephone, in person, telehealth and emails. Supports targeted intervention from program staff. Assists in follow up contacts, outreach, and updates. Provides staff training and education. Provides support in Integrated Health Clinics, CHA clinics, in member (or family) homes and in the community. Connects with member, family member and teams. Assists members in determining the objectives needed to reach their recovery goals. Instills hope in individuals by being a role model of recovery. Models effective coping techniques and self-help strategies. Provides and advocates for effective recovery-based, person-centered services. Interact with members and case managers Answer inbound telephone calls Maintain confidentiality of records relating to clients' treatment Fill out and maintain client-related paperwork, including federal- and state-mandated forms, client diagnostic records, and progress notes Prepare and maintain required treatment records and reports Coordinate member care in the service area Interact with consumers to obtain information for initial contact and set intake appointment. Exudes characteristics of compassion, empathy, and collaboration for connection with participants, family members and staff. Perform all other office tasks
POSITION REQUISITES
Qualifications and Experiences:
Required:
Has received or is currently receiving behavioral health services. High school diploma or equivalent required. Peer Support Specialist Certification required. Minimum of six months experience with clients in a behavioral health setting. Valid driver's license. Valid CPR/First Aid certifications or ability to obtain within 30 days of hire. Valid Fingerprint Clearance Card or ability to obtain within 90 days of hire. Three-year Motor Vehicle Record that meets our auto insurance carrier's coverage requirements.
WORKING CONDITIONS
: Physical requirements: Able to cope with emotional demands as they arise. Able to sit for extended periods of time. Able to hear ordinary conversation and phone communications.

LINES OF AUTHORITY
This position reports to the designated Program Manager and or Regional Director. This position provides outpatient services.
