Job Description

Job Overview Positive Growth, Inc. is seeking a dependable and experienced Human Services Professional (HSP) to support youth in our residential child-care program. The HSP plays a key role in ensuring that each youth's safety, permanency, well-being, educational, medical, behavioral, and family-service needs are addressed in accordance with agency policies and applicable state requirements. Primary Responsibilities Maintain regular individual contact with assigned youth and assess their safety, well-being, adjustment, and service needs. Participate in the development, implementation, and monitoring of Individualized Service Plans (ISPs) and treatment goals. Coordinate medical, dental, behavioral health, educational, and other required services. Monitor completion of required physical examinations, dental appointments, immunizations, and follow-up recommendations. Communicate regularly with DFCS case managers, legal guardians, therapists, schools, medical providers, families, and other members of the youth's treatment team. Maintain accurate and timely case documentation, monthly summaries, contact notes, assessments, service plans, and other required records. Participate in family/team meetings, school meetings, court-related activities, and case reviews as needed. Assist youth with permanency goals, independent-living skills, educational progress, family relationships, and preparation for adulthood. Identify concerns or changes in a youth's behavior, functioning, placement stability, or safety and communicate them promptly to appropriate team members. Ensure required services and documentation are completed within established timelines. Work closely with residential staff to promote consistency between each youth's service plan and daily care. Participate in crisis response, safety planning, and discharge/transition planning when necessary. Maintain confidentiality and comply with agency policies, Participate in required training, supervision, staff meetings, and quality-assurance activities. Perform other related duties necessary to support the children and the residential program. Qualifications Bachelor's degree from an accredited college or university in social work, psychology, sociology, counseling, human services, child and family development, criminal justice, or a related human-services field preferred. Relevant experience working with children, adolescents, families, behavioral health, child welfare, or residential services. Must possess strong written documentation, communication, organization, and time-management skills. Must be able to work effectively with youth who may have experienced abuse, neglect, trauma, behavioral challenges, or disrupted family relationships. Must demonstrate appropriate professional boundaries and sound judgment when working with youth. Must be able to manage multiple deadlines and maintain accurate case records. Must successfully complete all required background checks, health screenings, orientation, and agency/state-required training. Valid driver's license and acceptable driving record required. This position requires someone who will actively follow through on the needs of the youth assigned to them rather than simply identifying or documenting those needs.
Pay:
$20.00 - $21.00 per hour
