Position Summary Brighten Your Horizons, LLC is seeking a compassionate, dependable, and professional Individual Supports Specialist to provide one-on-one support to a 50-year-old male receiving NC Medicaid 1915(i) Individual & Transitional Supports (ITS). The Individual Supports Specialist will assist the member with developing independent living skills, increasing community involvement, improving social interactions, maintaining emotional wellness, and achieving goals identified in the Person-Centered Plan. Services are provided in the member's home and throughout the community under the supervision of a Qualified Professional. ITS focuses on building and strengthening skills for independent community living rather than performing tasks for the member. Essential Job Duties Provide one-on-one skill-building and coaching based on the Person-Centered Plan. Support the member with budgeting, money management, meal planning, grocery shopping, and other independent living skills. Encourage personal hygiene, organization, and household management. Assist the member in accessing community resources, recreational activities, and social opportunities. Promote healthy decision-making, self-advocacy, communication skills, and problem-solving. Provide support with transportation to approved community activities and appointments when authorized. Encourage medication adherence as directed by the care team (without administering medications unless appropriately credentialed). Document all services accurately and submit progress notes in a timely manner. Communicate significant changes or concerns to the Qualified Professional immediately. Maintain confidentiality and comply with HIPAA, Medicaid, and agency policies. Respond appropriately during behavioral or medical emergencies according to agency procedures. Attend required trainings, supervision meetings, and staff meetings. Minimum Qualifications High school diploma or GED required. Valid North Carolina driver's license, reliable transportation, and proof of automobile insurance. Ability to pass state and federal background checks. Current CPR/First Aid certification (or willingness to obtain). Strong communication, organizational, and documentation skills. Experience working with adults with behavioral health needs, intellectual/developmental disabilities, or substance use disorders is preferred. Preferred Skills Patient, dependable, and compassionate. Ability to build positive professional relationships. Strong problem-solving and crisis de-escalation skills. Ability to work independently with minimal supervision. Comfortable working in community settings.

Pay:

$22.66 - $27.28 per hour
