What You'll Do:

Connects residents and home-based non-residents to appropriate supportive services and assists with transitions to higher levels of care when needed Monitors the frailest residents through regular contact and post-hospital/therapy transitional care Develops and maintains a resource guide, and produces monthly service reports and annual HUD reporting Creates communication tools (newsletter, website, social media, marketing materials) and serves as liaison to resident groups Coordinates socialization opportunities, including events, classes, spiritual resources, and vocational/educational programming, to reduce isolation Works closely with management to ensure residents meet lease obligations and remain in compliance with HUD requirements

What You'll Need Required:

High School Diploma or GED 3+ years of experience in services for the elderly, disadvantaged, or social services in healthcare setting

Preferred:

Bachelor's Degree 5+ years of experience in services for the elderly, disadvantaged, or social services in healthcare setting Benefits Available to
