\nJefferson County's Comprehensive Community Services (CCS) program within the Human Services department provides a flexible array of individualized community-based psycho-social rehabilitation services for consumers with mental health or substance use disorders across their lifespan, with specific focus on children and adolescents for this position.

WHAT YOU WILL DO AS CCS SERVICE FACILITATOR I

\nThe purpose of this position is to provide psycho-social rehabilitative services to youth consumers diagnosed with mental health and/or substance-use disorders and enrolled in the Comprehensive Community Services Program. Services are guided by individualized recovery plans and are intended to support emotional, behavioral, and functional stability across community, home, and treatment settings. As a

CCS SERVICE FACILITATOR I

you will be successful in this role by completing the following tasks and responsibilities:\n

Facilitates and coordinates psychosocial rehabilitative services as specified in the Comprehensive Community Services Array (DHS 36), including screening assessments, service planning, service facilitation and follow-up care with consumers and their recovery team.\n
Monitors and supports physical health and wellness management as part of integrated service delivery.\n
Ensures consumer participation in the team-developed service plan and supports consumers with identifying, working toward, and achieving goals.\n
Completes initial and ongoing assessments within required CCS timelines to support service planning and evaluate consumer progress.\n
Collaborates with both consumer and recovery teams to collect information about consumers' family mental health and substance use histories.

Assesses information and provides recommendations for treatment and services to best support the consumers. Coordinates services between recovery team members including family, guardians, foster homes, providers, schools, churches, hospitals, etc.\nDevelops minimal mastery and ability to teach evidence-based practices, including CBT, DBT, and Skills System. Attends training to improve and reinforce skill sets.\nDually trained in Emergency Mental Health (EMH).\n

Requirements:

Bachelor's degree in social work or related field.\nOne year of related experience and/or training working with children, adolescents, and families in a clinical or community-based setting.\nExperience delivering or participating in evidence-based models\nValid Driver's License.\nMust pass the caregiver background check.\n
