07/13/2026 POSITION SUMMARY The Youth Services Program Support Coordinator supports youth in building skills, confidence, and independence through individualized services, mentorship, and community connections. This role works closely with youth, families, and staff to promote positive outcomes and program success.
ESSENTIAL JOB FUNCTIONS
Provide orientation and onboarding to youth and families entering program services. Complete intake assessments and individualized plans based on each participant's strengths, goals, interests, and identified needs. Develop, implement, and monitor individualized support plans that encourage youth development and successful achievement of personal goals. Serve as a resource and consultant to program staff and mentors regarding youth engagement strategies, trauma-informed practices, and positive behavior supports. Act as a liaison between youth, families, schools, community partners, service providers, and other stakeholders. Supervise, coach, and mentor program staff. Participate in the recruitment, training, and development of mentors and volunteers. Meet regularly with youth, families, and mentors to identify needs, monitor progress, and support goal achievement. Coordinate referrals and connect youth and families with community resources and support services. Understand and implement contract specifications, grant requirements, and program standards required by funding sources. Maintain accurate case records and ensure documentation meets quality and compliance standards. Support youth participation in educational, vocational, recreational, and community-based activities. Respond promptly to emergency situations and follow agency policies and procedures related to youth safety and well-being. Maintain the confidentiality of youth, families, staff, and Ascend Services business information at all times. Perform other duties as assigned.
JOB REQUIREMENTS
Must successfully pass pre-employment drug screening, physical examination, and TB testing. Must pass all required caregiver, background, and child welfare screenings. Availability to work evenings and other hours as required by program needs. Valid driver's license with a good driving record preferred. Must meet Ascend Services' vehicle insurability standards for operating company or personal vehicles when transporting youth.
EXPERIENCE AND EDUCATION
Bachelor's Degree in Human Services or a related field, or equivalent experience in place of formal education. Experience working with youth, families, or vulnerable populations required. Experience in mentoring, case management, youth development programs, education, behavioral support, or community-based services preferred. SKILLS Strong leadership, coaching, and organizational skills. Excellent verbal and written communication skills. Ability to build positive relationships with youth, families, staff, and community partners. Strong problem-solving, conflict resolution, and decision-making abilities. Knowledge of positive youth development principles and trauma-informed practices. Ability to work effectively with individuals from diverse backgrounds. Commitment to empowering youth and supporting their personal growth and success. Proficiency with Microsoft Office and electronic recordkeeping systems.
SUPERVISORY RESPONSIBILITIES
Direct supervision of program staff, mentors, and volunteers as assigned.
TYPICAL PHYSICAL DEMANDS
Ability to stand, walk, and participate in community activities for extended periods. Ability to lift up to 50 pounds with assistance. Frequent bending, reaching, squatting, and kneeling. Ability to transport and accompany youth during community, recreational, and program activities.
WORKING CONDITIONS
Services are provided onsite, in the community, and other youth-centered settings. Regular local travel may be required to support youth activities, meetings, and community partnerships. $46,000.00 - $49,000.00 per year
