Schedule & Pay Schedule:
Monday, Tuesday, Wednesday, Friday, and every other weekend | 7:00 a.m.-3:00 p.m.
Evening & Weekend Differential:
Additional $1.50/hour for hours worked during evenings and weekends. Make a Difference Every Day Lutheran Social Services of Wisconsin and Upper Michigan is seeking a Human Services Professional to join our growing In-Home Recovery Services (IHRS) team. In this community-based role, you'll support adults enrolled in Comprehensive Community Services (CCS) and Community Support Program (CSP) through Northcentral Healthcare, helping individuals achieve their recovery goals and live more independently. This is a continuous posting in anticipation of future full-time, benefits-eligible opportunities. Essential Duties & Responsibilities This list is not intended to be all-inclusive and may be expanded as business needs require. Provide medication management and recovery support in homes and community settings. Collaborate with clients, families, and multidisciplinary treatment teams. Participate in recovery planning and service coordination. Maintain accurate and timely documentation in accordance with agency and regulatory standards. Apply counseling, case management, and community resource knowledge to support client goals. Attend required trainings, supervision, and staff meetings. Respond to client needs, including occasional evening and weekend services. Perform other duties as assigned. Valid driver's license, reliable transportation, and ability to meet LSS driving requirements. Strong communication, organizational, and documentation skills. Proficiency with computers and electronic health records. Preferred Experience providing community-based behavioral health, case management, or human services. Experience working with individuals experiencing significant mental health concerns. Work Environment This position is community-based and requires frequent travel throughout the Wausau area. The role includes working in client homes and community settings and may require bending, kneeling, climbing stairs, and responding to crisis situations. LSS is an Equal Opportunity Employer (EOE)
