JOB SUMMARY

The Community Support Specialist is responsible for providing access to services in the community and to individuals with serious and persistent mental illness(es) and individuals with other co-occurring disorders such as substance abuse disorders, developmental disabilities, and other co-occurring problems that impact life functioning. Community Support services focus on helping individuals develop life skills, access resources and learn to manage illness in order to be successful in the living, working, learning, and social environments of their choice. Skills and supports are addressed on an individualized treatment plan.

MAJOR JOB ELEMENTS

• Present client cases in Supervision.
• Document services that clearly describe the need for the service, the intervention provided, the relationship to the treatment plan, the provider of the service, the date, actual time and setting of the service, and the individuals response to the service.
• Work collaboratively with the individual on treatment goals and service including the use of collaborative documentation as a tool to ensure that individuals are active in their treatment.
• Develop, implement, update, and revise, as needed, the treatment plan that identifies specific, measurable and individualized interventions. This includes participating in treatment plan staffing and reviews.
• Develop a discharge and aftercare/continuing recovery plan to include, if applicable, securing a successful transition to continued services.
• Provide information and education to the individuals, families, and significant others regarding the types, availability and places of services which may assist in the individual's rehabilitation. This includes locating and helping clients utilize community resources.
• Maintain a professional working relationship with other agencies as well as collaboration with consumer services provided within CHS (i.

• Monitor and assist as needed with individual's medical concerns.
• Participate in CSS crisis on-call service by supporting individuals in crisis situations including locating and coordinating resources to resolve the crisis.
• Provide assistance to individuals in a responsible and flexible manner. Demonstrate ability to prioritize consumer needs. Maintain contact with individuals who are hospitalized for medical or psychiatric reasons and participate in and facilitate discharge planning for psychiatric hospitalization and for medical hospitalizations as appropriate.
• Provide training in activities of daily living skills (i.e. housekeeping, cooking, personal grooming.)
• Provide individualized assistance to consumers to create a personal support system that includes family members, legal guardians, and/or significant others.
• Work with supervisor to establish goals to meet organizational expectations and consumer needs and be accessible during regular office hours.
• Maintain cleanliness of the work environment.
• Participate in chart reviews as assigned and make necessary corrections.
• Successfully meet and maintain all certification, training, and accreditation requirements as deemed necessary by the organization.
• Meet all organizational billing expectations as well as timely submission of SDL's and progress notes.

QUALIFICATIONS

• Graduate from an accredited college or university with a Bachelor's Degree in social work, counseling, psychology, or closely related field; OR
• At least four years of relevant, qualifying full-time equivalent experience in human service delivery and demonstrates skills in developing positive and productive client relationships and the ability to negotiate complex service systems to obtain needed services and resources for clients; AND
• Be at least 23 years of age.

COMPENSATION

• Starting base rate of $19.

00/hour, commensurate with experience, with eligibility for an attractive bonus structure that rewards performance.
