Job Description

Job Overview The Community Programs Mentor- Le Cirque provides goal-directed, highly structured mentoring services for assigned youth and is responsible for implementing each youth's service plan and assisting each youth in meeting his/her program goals. The role of the Community Programs Mentor- Le Cirque is to provide services aimed at preventing/reducing youth involvement in oppositional behavioral patterns and providing goal-directed, structured, academic tutoring, social skills building, recreational, and artistic expression in an after-school setting, during holidays from school, and during summer breaks to children residing in Terrebonne Parish.
Essential Job Functions:
Develop and maintain positive mentoring relationships, acting as a positive role model for youth through one-on-one outings that are gender and developmentally appropriate for each assigned youth, according to his/her interest. Youth shall be mentored and assisted with homework, career exploration, and relationship building, as described in the service plan. Provide face-to-face contact with assigned youth, in accordance with each youth's service plan, contract requirements, and programmatic policies and procedures. In collaboration with the youth's Multidisciplinary Team, develop a written service plan for each youth upon admission into the program and provide the youth with all required services and activities prescribed in the plan. Ensure all services are performed in accordance with established Agency, programmatic, contract, and licensing/regulatory requirements, procedures, and deadlines. Develop and maintain positive relationships with assigned youth and their families. Ensure accurate documentation of assigned youth's records. Complete all documentation and reports in compliance with contract and programmatic requirements. Maintain youth records and ensure their safety and confidentiality. Abide by Agency policies on confidentiality. Maintain accurate and appropriate activity and travel logs, as prescribed by Agency and programmatic policies and procedures. Report assigned youth's progress and accomplishments in a timely manner, as required by programmatic policies and procedures. Appropriately report all suspected violations of an assigned youth's rights, including the infliction of abuse and neglect. Report all critical incidents in accordance with established reporting procedures as per program policies and procedures. Provide transportation for community outings outlined in their service plan, as required. Regular attendance is required, as scheduled.
Other Responsibilities:
Develop and maintain positive relationships with clients and their family members; promote cooperative teamwork. Attend and participate in all scheduled training sessions for Mentors Perform other job-related duties, as assigned.
Knowledge, Skills and Abilities:
High school diploma or equivalent with experience working with children. Teaching experience is preferable. Good verbal, written, interpersonal, and organizational skills. Patience, empathy, dependability, and the ability to teach others. Represent the Agency in a professional manner. Maintain confidfentiality of all client information. A valid driver's license per Louisiana law, access to a reliable vehicle in safe operating condition with current registration and current liability insurance.
