Job Description

Become a part of our caring community Humana's In-home Health & Well-being Assessments ( IHWA ) Program is a well-established, high-performing initiative designed to deliver comprehensive health assessments to Medicare Advantage members. The program supports member education, closes Stars gaps, and facilitates timely follow-up for unmanaged medical, behavioral, and social determinants of health ( SDOH ) needs. The IHWA department is structured around the core priorities outlined in the IHWA five-year strategic plan, including Member Engagement, Volume Performance, Stars Support, Post-Visit Follow-Up, Program Value, and Vendor Operational Excellence. The IHWA Performance Lead is responsible for proactively tracking, analyzing, and managing performance across cohorts, states, and vendor partners. This role collaborates closely with internal stakeholders and external vendors to identify performance trends, surface risks and opportunities, and ensure outcomes align with program and bid objectives. The IHWA Performance Lead designs and drives performance management strategies that support value-based program outcomes. This role partners closely with senior leaders on implementation planning, performance optimization, and continuous improvement initiatives across cohorts, states, and vendors. The Performance Lead analyzes and communicates program results, identifies trends and risks, and provides data-driven insights to inform decision-making. The role helps develop functional and segment-specific strategies on complex and high-impact performance matters. The Performance Lead exercises independent judgment and discretion in addressing multifaceted performance challenges, determining appropriate courses of action, and recommending corrective or enhancement strategies. This position operates with minimal supervision and requires advanced analytical thinking, effective stakeholder collaboration, and sound decision-making across variable and dynamic conditions.
