Job Description

Community Physicians is a well-established, patient-centered healthcare group dedicated to providing high-quality, compassionate care throughout Illinois. Job Summary Seeking a highly motivated and experienced individual who has management experience to oversee the daily operations of Longitudinal Care CCM Programming within the Care Navigation Department of CHP. This role will function under the supervision of the Director of Care Navigation. The role requires strong leadership skills and the ability to collaborate with various departments and staff to achieve department and company goals. Responsibilities Below are the roles and responsibilities that the Manager is expected to fulfill to help facilitate a productive department of Case Manager and Care Coordinators.
  • Monitor daily operations, employee workflow, and ensure compliance with department and company standards.
  • Manage and supervise in-office and remote staff.
  • Work alongside the department's Director to develop, implement, and maintain new projects and workflow.
  • Lead, mentor, and train staff to foster a positive and supportive work environment and promote professional development.
  • Analyze performance metrics of Nurse Case Managers and Care Coordinators and prepare reports for senior management to inform decision-making.
  • Ensure effective communication across all levels of the organization to facilitate teamwork and collaboration.
  • Ensure patient satisfaction by monitoring the team's work for completion, efficiency, and accuracy.
  • Evaluate department staff's performances and conduct employee reviews.
  • Assign/delegate appropriate tasks to department employees as needed.
  • Conduct regular meetings with staff to ensure the team is up to date on department and company wide changes.
  • Other duties as assigned. Skills
  • Proven experience in operations management or a related field.
  • Self-motivated and able to work both independently and in collaboration with others.
  • Strong analytical skills with the ability to interpret data and make informed decisions.
  • Ability to communicate proficiency through technology and maintain accurate notes and records.
  • Ability to maintain confidentiality of all medical, financial, and legal information.
  • Excellent leadership abilities with a focus on team development and motivation.
  • Exceptional communication skills, both verbal and written.
  • Ability to thrive in a fast-paced environment while managing multiple priorities.
