Glance:

Build and nurture referral relationships with hospital discharge planners, case managers, physicians , and other providers Collaborate with admissions and clinical teams to ensure smooth and appropriate placements Actively manage the referral pipeline and follow up with professionalism and care Track and report performance metrics regularly—support is provided, but ownership is expected Represent Vermont Healthcare Center with authenticity, warmth, and professionalism

What We're Looking For:

5+ years of skilled nursing or post-acute care marketing experience A strong referral network and proven ability to grow census Working knowledge of Medicare, Medi-Cal, managed care, and HMO processes Excellent communication, organization, and follow-up skills A collaborative, goal-driven mindset—you're self-motivated but team-oriented
