Do you enjoy working in a service-oriented industry? If so, this opportunity may be the right fit for you! Modivcare is looking for an experienced Facility Outreach Manager to join our team. In this role, you will serve as an advocate within the non-emergency medical transportation (NEMT) system by supporting patients, case managers, and healthcare facilities through outreach, education, relationship management, and coordination efforts. The ideal candidate is relationship-driven, strategic, highly organized, and passionate about improving transportation access and healthcare outcomes. This role… Advocates within the non-emergency medical transportation system for patients and case managers. Serves as an advocate for organizational goals and initiatives within the community. Provides in-service training to healthcare facilities on how to effectively use NEMT services and promote positive relationships. Ensures facility outreach and training activities are conducted throughout the assigned territory. Coordinates community-based outreach, event planning, and educational initiatives. Develops and implements outreach strategies to strengthen partnerships and stakeholder engagement. Builds and maintains ongoing relationships with facilities through face-to-face meetings, phone calls, and group presentations. Analyzes subscription service requests (standing orders), providing outreach and education to improve quality and resolve issues. Reviews patient attendance information and transportation trends from healthcare facilities. Analyzes operational data and develops cost management strategies. Oversees facility coordination and quality assurance activities in partnership with operational leadership. Identifies and contracts with Sole Source Transportation Providers, including rate negotiation and end-to-end contract management. Leads facility networking, tracking, and reporting outreach efforts. Collects and analyzes feedback from community partners and stakeholders to identify opportunities for improvement. Provides TripCare portal training and development support to facilities and stakeholders. Provides guidance and support to junior team members. Participates in special projects and additional duties as assigned. Requires business travel up to 80%. We are interested in speaking with individuals with the following… Bachelor's Degree required. Five (5) plus years of experience in member services, transportation services, utilization review management within a health plan, or social work experience. Medical transportation management experience preferred. Experience with facility-based social service programs, including discharge planning, preferred. Clinical, case management, and discharge planning experience preferred. Or equivalent combination of education and/or experience. In-depth knowledge of healthcare facilities, medical groups, clinical case management, and discharge processes. Familiarity with facility-based social service programs and discharge planning. Strong verbal, written, and interpersonal communication skills. Ability to effectively communicate with clients and providers and deliver public presentations. High level of technical proficiency with Microsoft Office Suite; advanced Excel skills preferred. Excellent organizational, analytical, and problem-solving skills with the ability to develop and execute action plans. Proactive approach to relationship building and stakeholder engagement. Solutions-oriented mindset with strong creative and innovative thinking abilities. Ability to work independently and collaboratively within a team environment. Strategic thinker with the ability to anticipate operational and organizational needs. Ability to manage multiple priorities and projects simultaneously. Knowledge of clinical utilization review, quality assurance, Medicaid guidelines, and covered services. Working knowledge of Medicaid and Medicare programs. Excellent leadership and collaboration skills.
Salary:
