Assisted Living Facility Job Description:
We are seeking a compassionate and dedicated Memory Care/Resident Care Coordinator for our assisted living facility. The ideal candidate will have a passion for working with individuals in need of memory care and ensuring they receive the highest quality of life possible in a supportive environment with activities throughout the day.
Responsibilities:
Coordinate and deliver comprehensive care plans tailored to meet the individual needs of residents in the memory care unit. Collaborate with healthcare professionals, caregivers, and families to ensure optimal memory care support. Maintain and oversee the implementation of medication management and daily living assistance for residents. Develop engaging and therapeutic programs to enhance the cognitive and emotional well-being of residents. Conduct routine assessments and observations to monitor residents' progress and adapt care plans as necessary. Provide training and mentorship to caregiving staff to ensure the highest standard of care is delivered. Foster a warm, safe, and nurturing environment that respects the dignity and preferences of each resident. Act as a liaison between families and the facility, offering updates on residents' health and contributing to family meetings. Ensure compliance with all state and federal regulations pertaining to assisted living and memory care services. •
Qualifications:
•Experience in a similar role within assisted living or memory care settings. Knowledge of dementia and Alzheimer's disease and the best practices for their management. Excellent communication, leadership, and organizational skills. Ability to work collaboratively with a diverse team of caregivers and healthcare professionals. Certification in gerontology, nursing, or a related field is preferred. A compassionate and patient-centered approach with a commitment to providing exceptional care to residents.
