Responsible for the complete referral process, including identifying home health needs, verifying benefits, and coordinating care. Cultivate new and existing relationships resulting in company growth. Initiate and expand referral network. Comply with all aspects of the company's compliance and HIPPA regulations. One year of home health, marketing, or case management experience or a combination of each. Must possess a valid driver's license.

Compensation:

$75,000-$85,000 plus bonus
