Job Description

Currently hiring full-time Care Manager
  • Extender to assist in providing comprehensive care management services for individuals with I/DD.
The Care Manager Extender will be responsible for providing whole person care management support to the Care Manager by assisting with the coordination and monitoring that focuses on individual identified needs.
Essential Duties :
  • Performing general outreach, engagement, and follow-up with members
  • Facilitate communication and coordination between members of the care team, and involve the member in the decision-making process, to coordinate needed care and services.
  • Engaging in health promotion activities and knowledge sharing.
  • Providing and tracking referrals and providing information and assistance in obtaining and maintaining community-based resources and social support services.
  • Support the care manager in assessing and addressing unmet health-related resource needs.
Qualifications:
1. Has a high school diploma, College, or equivalent. 2. Familiarity with healthcare laws, regulations, and standards. 3.
Skills:
Demonstrated leadership ability. Ability to communicate effectively orally and in writing. Analyzing information, Strategic planning, Attention to detail, thoroughness, dealing with Complexity. 4. Case management experience (Preferred) Salary is commensurate with education and experience.
