Job Description
Incumbent works under the direction of the Director of Outpatient Services or designee, serving as a primary source of information and referral, and assisting program participants in accessing needed social services and care from other community providers. Provide client case management services (e.g. entitlements, housing, medical services, social support), and schedule appointments with other care providers. Establish and maintain effective work relationships with clients, supervisors, County employees, elected officials, attorneys, law enforcement, judges, other agencies, and the public. Comply with Civil Service rules and regulations, collective bargaining agreements, County policies, department policies and laws to create a cooperative, safe, respectful and quality work environment. Maintain and process records and record communication in accordance with standards established by CMHC, local, state, and federal accreditation guidelines ensuring proper documentation is performed. Document case management services in the Electronic Health Record (EHR) system. Ensure services comply with professional standards, state, and federal regulations and policies and procedures. Receive cross-training for other CMHC programs and provide services within these programs. Work independently in the community, meeting with participants in community settings, providing transportation, and participating in appointments. Serve as a liaison between outside social service agencies and CMHC Outpatient programs. Schedule conferences with clients and families per treatment team's directive. Maintain assigned caseload and productivity based on CMHC criteria. Participate in the development of policies and procedures, CMHC committees, treatment team and staff meetings, and other related groups. Perform quality assurance activities. Maintain knowledge and job skills (e.g. research, meetings, seminars, webinars, continuing education). Monitor Board of Mental health committed patients for compliance with medication and appointments and provide required updates. Safely operate a motor vehicle when required to travel on County business. Report to work with regular consistent attendance. Perform other duties as assigned and directed. Bachelor's degree from an accredited university or college in Social Work, Psychology, Guidance & Counseling, Sociology, or Human Services required.
- One (1) year of human or social services casework experience required.
- Valid driver's license upon hire and maintained throughout employment required. Completion a pre-employment criminal record check, physical assessment, and conditional offer drug screen required.
- Equivalent combination of work experience and education may be substituted for requirements on a year-for-year basis.
