Job Description
Regional Grant Coordinator . Only work authorizations not requiring sponsorship now or in the future will be considered. Candidates must be based in Shelby, Rush, Decatur, Franklin, Jennings, Ripley, Dearborn, Scott, Jefferson, Switzerland, Ohio. May need to travel to Indianapolis on occasion for mandatory in-person meetings (reimbursement provided). Work schedule is 37.5 hours per week. What You'll Do + Participate in regular check-ins, statewide meetings, and required trainings Regional Grant & Contract Management + Serve as the primary regional contract manager for organizations receiving RHTP funding + Monitor compliance with state and federal requirements, including allowable expenditures, budget adherence, and documentation standards + Support IDOH in ensuring regional coalitions meet milestones, deliverables, and timelines outlined in the state-defined workplan + Identify risks, performance issues, or compliance concerns and escalate to IDOH as appropriate + Maintain audit-ready documentation and ensure consistent use of state-approved templates, tools, and reporting systems •Coordination with Technical Assistance Providers (IHA, IPHCA, IRHA)•+ Collaborate closely with statewide Technical Assistance Providers to ensure aligned support for regional coalitions + Participate in regional meetings convened by Technical Assistance Providers, including consortium meetings, workgroups, and application development sessions + Facilitate cross-region learning by sharing insights, challenges, and best practices •Oversight of CMS Reporting Requirements•+ Coordinate regional data collection, performance tracking, and outcome measurement in alignment with CMS Rural Health Transformation Program requirements + Ensure timely submission of all required regional reports to IDOH for state and federal reporting cycles + Support regional partners in understanding and meeting CMS reporting expectations, including documentation, metrics, and narrative requirements •Regional Stakeholder Engagement & Communication•+ Serve as the day-to-day liaison between IDOH and regional coalitions, including hospitals, clinics, EMS providers, community organizations, and other partners + Promote broad representation of rural populations and ensure regional activities reflect local needs and priorities + Maintain strong communication channels with IDOH's Regional Grant Initiative Team and participate in statewide meetings as requested •Program Implementation Support•+ Assist regional partners in interpreting state guidance, funding requirements, and program expectations + Track regional progress, identify barriers, and support problem-solving to maintain momentum + Contribute to statewide consistency by using shared tools, templates, and evaluation frameworks •What You'll Need•

These grants represent a multi-year investment to strengthen rural healthcare access, prevention, and system resilience across Indiana. This position will be

Required:
+ Bachelor's degree in public health, health administration, public policy, business administration, or related field + 5 years of experience in grant management, contract oversight, program administration, or related work + 3 years of experience working with healthcare organizations, rural communities, or public health systems + 2 years of experience with compliance, budgeting, and reporting requirements for state or federal grants + Ability to manage multiple projects simultaneously with strong attention to detail + Excellent communication, facilitation, and relationship-building skills + Ability to work independently as a remote field-based contractor + Must reside within the assigned RHTP region and be willing to travel regularly within the region + Experience with CMS-funded programs or federal cooperative agreements + Familiarity with Indiana's rural health landscape, including hospitals, FQHCs, EMS, and community-based organizations + Experience collaborating with statewide associations or multi-stakeholder coalitions + Knowledge of rural health delivery, care coordination, or health system transformation- Physical Demands
